Annex IX — Information to be Submitted to the EU Database pursuant to Article 49(3)

Information to be submitted by providers of high-risk AI systems covered by Union harmonisation legislation listed in Annex I:

1. Name, address and contact details of the provider.
2. Name, address and contact details of the authorised representative.
3. AI system trade name and any additional unambiguous reference allowing identification.
4. Description of the intended purpose.
5. Version number of the AI system.
6. Status of the AI system.
7. Identification number of the notified body (where applicable).
8. Number of the certificate (where applicable).
9. Member States where the AI system is placed on the market, put into service or made available.
