Annex VIII — Information to be Submitted to the EU Database pursuant to Articles 49(1) and (2)

Section A — Information to be submitted by providers of high-risk AI systems listed in Annex III (not covered by Annex I):

1. Name, address and contact details of the provider.
2. Where submission is carried out by another person on behalf of the provider, the name, address and contact details of that person.
3. Name, address and contact details of the authorised representative, where applicable.
4. AI system trade name and any additional unambiguous reference.
5. Description of the intended purpose and the categories of natural persons and groups likely to be affected.
6. Status of the AI system.
7. A basic description of the data used for training (main characteristics, sources, and nature).
8. Post-market monitoring plan reference.
9. The URL for additional information (optional).
