Annex X — Information to be Submitted to the EU Database pursuant to Article 49(4)

Information to be submitted by deployers of high-risk AI systems referred to in Annex III, point 2:

1. Name, address and contact details of the deployer.
2. Name, address and contact details of the provider of the AI system.
3. AI system trade name and any additional unambiguous reference.
4. Description of the intended purpose for which the AI system is used.
5. Member State(s) where the AI system is deployed.
6. A concise description of the fundamental rights impact assessment, where applicable.
7. URL for additional information (optional).
