EU Declaration of Conformity

Annex V — EU Declaration of Conformity referred to in Article 47

The EU declaration of conformity shall contain all of the following information:

1. Name and address of the provider or their authorised representative.
2. A statement that the EU declaration of conformity is issued under the sole responsibility of the provider.
3. Name, address and identification number of the notified body (where applicable).
4. Trade name or brand, address and type or identification number of the AI system.
5. Statement that the AI system is in conformity with this Regulation.
6. Where applicable, a reference to relevant harmonised standards used, or to other common specifications in relation to which conformity is declared.
7. Where applicable, the name and identification number of the notified body, a description of the conformity assessment procedure performed and identification of the certificate issued.
8. Place and date of issue of the declaration.
9. Signature and name/function of person signing.
